Dance Manitoba Festival 2010: Entry Form

Official entry forms are available at the Dance Manitoba offices or you can download the form, fill it in on-screen if you like, then print it off and submit the completed form with your payment. You can also print the blank form and fill it in manually, but if you fill it in on-screen it is clearer for us to read. After printing it out, you can try to submit the form to us via email attachment, but you will be required to sign the form at our office before it is valid.

You can download the entry forms here in PDF format. Please note that TWO completed copies of the form must be submitted for each entry along with the fees.

You must also download the RULES book as you will be required to agree to the details therein. If you require the free Acrobat Reader software to view and print the file, please download the current version from Adobe.
